Membership Hold Request

  • Holds will only be granted for medical reasons (doctor note may be required).

  • Holds must be requested in writing (form below) within 3 days of incident.

  • Hold will begin on the date of the last attended class OR when the hold request is received if later than 3 days after the incident.

  • It is the responsibility of the member to notify admin of their return date if it is different from the date written on the request form. A minimum of 2 business days notice is required.

  • Extension requests will be granted on a case by case basis.

Please give notice of 2 days prior to return to adjust return date. If date needs to be extended, please notify admin at least 2 days before the date entered on this form. Extension requests will be assessed on a case by case basis.
